Session Chronicles Step-by-Step Methodology to Build Clinically Integrated Networks

Frisco, Texas – March 15, 2016 – Conifer Health President of Value-Based Care, Megan North and Amanda Skinner, executive director of Clinical Integration and Population Health for Yale New Haven Health System (YNHHS), will co-present “A Step-by-Step Approach to A Clinically Integrated Network,” to attendees of the 2016 American College of Healthcare Executives’ Annual Congress on Healthcare Leadership (ACHE Congress). The 2016 ACHE Congress runs March 14-17 at the Hyatt Regency in Chicago. North and Skinner will deliver Session 78X from 8:45-10:15 a.m. CDT on March 16.

YNHHS is one among a number of large health systems that have engaged with Conifer Health to assist in structuring risk models to support effective value-based care arrangements. North and Skinner will discuss elements associated with building a clinically integrated network including legal, clinical and financial considerations. The co-presenters will also demonstrate how accurate data helps benchmark the quality of care, utilization of resources, and cost efficiencies to develop a sustainable governance and incentive structure.

The joint Conifer Health-YNHHS session comes as a September 2015 survey conducted by HealthLeaders Media Intelligence, found that nearly 52 percent of hospitals and health systems executives surveyed said they see clinical integration as the cornerstone for both the “strategic alignment of physicians in their respective care communities and to better manage the health of populations across a complete care continuum.”
