Yale New Haven Health System Teams with Conifer Health Solutions on Clinical Integration Network
Consulting Engagement Sets Foundation for Advanced Population Health Management

Conifer Health Solutions today announced a new value-based care consulting engagement with Yale New Haven Health System (YNHHS), one of the leading integrated delivery networks in the state of Connecticut. The consulting services arrangement will help YNHHS enhance collaboration among physicians and other providers; and lays the foundation for prevention and wellness management for populations across the state.

Under terms of the agreement, Conifer Health will partner with YNHHS to design and build a clinically integrated network. Conifer Health’s work will also set the stage for a comprehensive population health management strategy to optimize quality, safety and patient satisfaction.

Conifer Health will employ the combined strength of its value-based care services—Clinical Integration, Population Health Management and Financial Risk Management—to help YNHHS redesign its clinical networks.
